Looking to obtain or renew your DOT Medical Examiner's Certificate?

Common Sense Chiropractic is here to provide a quick and affordable physical examination to make sure you are in compliance with FMCSA standards. Whether you drive a bus or a semi, the most important factor is YOU! The purpose of this examination is to confirm that you are healthy and able to safely perform your duties as the operator of a Commercial Motor Vehicle. 

At the appointment we will complete the Medical Examination Report (MCSA-5875) and issue your Medical Examiner’s Certificate (MCSA-5876) if you qualify. Results will be submitted electronically to the National Registry upon completion.  

Here is a list of information needed to ensure your exam goes smoothly: 
  • Valid photo ID 
  • Medication list, including dosages 
  • Glasses or contacts, if needed, while driving 
  • If diabetic, the most recent A1C or lab work 
  • Sleep data if using a CPAP machine for sleep apnea 
  • Any required specialist letters or clearance forms
